Clinic Policies

Late Appointment Change Policy - To respect the time of our providers as well as the patients on the waiting list, if an appointment is cancelled/rescheduled less than 48 business hours before it is scheduled there will be a $100 fee. No-showing, leaving after your appointment started, or arriving over 15 minutes late to your appointment will incur the same fee.

Optomap Policy - For each annual eye exam, we assess the health of our patients' eyes, including their retinas. We perform Optomap retinal imaging for this purpose. This method is more effective than traditional dilation and does not affect the patient's vision. Please note that most vision insurance plans do not cover Optomap imaging, resulting in a $39 out-of-pocket charge.

If you prefer dilation, please inform our scheduling staff in advance and arrive 10 minutes early for your appointment. Due to time constraints, dilation may not be an option if you do not arrive on time.

Financial/Insurance Policy - As a courtesy we try to check our patient’s insurance benefits before their appointment, though knowing benefits, eligibility, and coverage is the patient's responsibility for any medical or vision appointment. If we do not receive a patients insurance information three business days in advance of their appointment, or if we are out of network with a patient's insurance plan, we will charge the visit as out of pocket and the patient can submit the itemized receipt/superbill to their insurance for potential reimbursement. All fees are due on the day of service. Please note that patients are responsible for all charges incurred; our verification of your insurance benefits is not a guarantee of payment.​​​​​​​

Outside/Online Eyewear Service Package - To help minimize potential problems when purchasing eyewear online or at discount retailers we offer a service package for $35 which includes a measurement of your pupillary distance (PD), which is the horizontal distance between your pupils and is needed to properly align the lenses. This is not part of your annual eye exam but part of the measurements taken when you are ordering eyewear. We also will analyze your glasses upon receipt and evaluate the eyewear for prescription accuracy and proper alignment of the PD. Addiitonally we will adjust your eyeglasses to fit properly both for comfort and best lens alignment. Regardless of where you purchase your glasses we want you to have the best possible fit!

ID Policy - As a medical facility, it is mandatory for patients to have a valid government identification (ID) on file before attending an appointment or making any purchase at Eyes on the City, including but not limited to glasses, contact lenses, medications, medical supplies, and other healthcare-related products or services. This policy is to ensure patient safety, prevent fraud, and maintain accurate records of transactions.

Clothing Policy - We are committed to maintaining a safe and hygienic environment for patients, staff, and visitors. As a healthcare clinic we require all patients to wear appropriate clothing and footwear while on the premises. This policy applies to all areas of the facility, including waiting rooms, examination rooms, treatment areas, and any other patient-accessible areas.

COVID Masking Policy - In accordance with the Washington State Department of Health (DOH) Health Mask Order update, effective as of April 3rd, 2023, Eyes on the City will discontinue required masking. We will provide masks for any patients who would like them. We also request that patients reschedule their appointments if they are experiencing any symptoms of illness.

Our staff will also not be required to wear masks, though some still elect to. If a patient desires, the provider and/or optician managing their care are more than happy to wear a mask at the patient's request. We require all staff to keep up with their covid and flu vaccinations. We also have a strict workplace illness policy, if a staff member is feeling any cold/flu/allergy symptoms, they are required to test and wear a mask regardless of results. If symptoms are minor and their covid test result is negative, they can remain at work while masked. If symptoms are moderate or worse they will be asked to go home for the duration of their symptoms.
